Booking Policy

 

OVERVIEW

This Booking Policy outlines the terms and conditions for scheduling and attending services with Gabriel Integrative Medicine. By booking an appointment through our website, you agree to the terms and conditions outlined below.

SECTION 1 – ONLINE-ONLY APPOINTMENTS

  • All services at Gabriel Integrative Medicine are provided exclusively through online (virtual) appointments.
    We do not offer in-person consultations or services at this time.

  • Appointments are conducted securely via HIPAA-compliant video conferencing platforms. Clients are responsible for ensuring they have a stable internet connection and appropriate technology to participate in their scheduled sessions.

 

SECTION 2 – APPOINTMENT REQUIREMENTS

To book a service, clients must:

  • Provide accurate and complete personal and medical information

  • Submit all required intake forms at least 48 hours before the scheduled appointment

  • Provide a valid debit or credit card at the time of booking to secure the appointment

  • Failure to complete intake requirements may result in appointment cancellation or rescheduling.

 

SECTION 3 – DIAGNOSIS & SCOPE OF CARE

Gabriel Integrative Medicine provides medical consultations that include diagnosis and treatment within the licensed scope of practice of our healthcare providers.

However, please note:

  • We do not diagnose or treat cancer.
    Our oncology-related services are supportive only, intended to promote overall wellness alongside your ongoing oncology care.

  • Clients with suspected or diagnosed cancer should be under the care of a licensed oncologist.

 

SECTION 4 – CANCELLATIONS & RESCHEDULING

We require at least 48 hours' notice to cancel or reschedule an appointment.

  • Cancellations made more than 48 hours before the appointment: No charge

  • Cancellations made less than 48 hours before the appointment: Subject to a $75 late cancellation fee

  • No-shows: Full service fee charged to the card on file

Multiple missed or late-canceled appointments may result in restricted access to future bookings.

 

SECTION 5 – PAYMENT POLICY

  • Payment is required at the time of booking.

  • We accept credit cards, debit cards, and HSA/FSA cards where applicable.

  • Services billed through wellness plans or packages still require a card on file to enforce the cancellation policy.

 

SECTION 6 – TESTING SERVICES & SHIPPING

Gabriel Integrative Medicine proudly partners with DUTCH (Precision Analytical Inc.) and AxGen (Applied Genetic Technologies) to provide specialty at-home testing kits.

  • All testing kits are fulfilled, shipped, and processed according to each laboratory's own policies and procedures.

  • Testing kits are non-refundable once shipped.

  • Any kit concerns or replacement requests are subject to approval by the respective laboratory.

 

SECTION 7 – LIMITATION OF SERVICES

While Gabriel Integrative Medicine provides comprehensive care in wellness, hormone health, and metabolic support, we do not provide:

  • Emergency medical services

  • Oncology diagnosis or cancer treatments

  • Surgical or invasive medical procedures

Clients are encouraged to maintain an active relationship with their primary care physician or appropriate medical specialists.

 

SECTION 8 – POLICY AGREEMENT

By booking any service through Gabriel Integrative Medicine, you confirm that you have read, understood, and agreed to this Booking Policy.
